How do you do continuous improvement in your workplace? Do you have a formal workgroup / committee? How do you collate and track the different units' continuous improvement efforts?

So many ways!
We have an Improvement Suggestion Form and we encourage staff to send their suggestions that way. We also encourage suggestions every day through incidental communication and supervision. 
We have a quality register where were record all improvement plans to make sure when action the ideas that come in. 
We consult with staff on changes throughout the process. 
We undertake staff survey, and aim to undertake participant surveys ASAP. 
There are so many things we want to do!
